Membership Application

You can apply for membership online or by mail. The Chapter looks forward to having you as a member.

Enter information into the Electronic Form and then click on "Submit" to submit your application electronically. Note: Payment should not be made through the Internet; send payment with the printable form, or the Chapter will bill you.

There are two ways to submit your membership application:

Electronic Format (Secure Online Application coming soon)
Click on the above link, complete the form and hit the "Join" button. Your application is sent immediately to the LA-ACS office. Print your confirmation page and mail to Chapter Headquarters with your check for the appropriate dues amount.

Printed Application (RTF)
Click on the above link for an application file that you can print, fill out and mail with your check for the appropriate dues amount.
